I authorize my child’s participation in the Green Meadow Summer Camp. It is my understanding that participation in the activities that make up the Green Meadow Summer Camp involves some inherent risk of injury.  As such, in consideration of my child’s participation in the Green Meadow Summer Camp, I hereby release Green Meadow Country Club, employees, Green Meadow Summer Camp staff, or anyone associated with Green Meadow Country Club from all responsibility or liability for any loss, damage, or injury, including death.  I also understand that I am responsible for health insurance coverage for my child. 

I also give my permission for any emergency, medical care, or treatment by a physician, hospital, surgeon, or medical care facility, and I accept responsibility for the cost.  In the event that such emergency care is needed, I give my permission for a member of the Green Meadow Summer Camp staff transport my child to receive such care. 

I also agree that I am financially responsible for any property damage caused by my child.  I also understand that any improper conduct or inappropriate behavior by my child may result in my child’s dismissal from the camp without refund. 

I certify that my child is in good physical condition to participate in the Green Meadow Summer Camp. 

I hereby state that I am the legal guardian of said child.

GMCC Junior Golf Champions -- December 2006

     Kudos go out to all of our Junior Club Championship participants for braving the elements and for finishing this years event. The first day rain out was followed by 40-degree temperatures and winds about the same number. Purple hearts were given to all players and trophies to the following division winners.

17-18 Justin Niethammer;             15-16 Tim Tipton wins in a playoff over Jonathan Jones;

13-14 Grant Dougherty;                 6 and under- Riley Sheets edges out Braden Wear.
